Millbrook Rise Centre is a 27-bed mental health subacute inpatient facility; we are a consumer-centred service with a focus on rehabilitation and recovery located in the beautiful Derwent Valley. We work closely with various services within not only Statewide Mental Health Services (SMHS) but also with the wider community services such as NDIS providers. We are a collaborative team that works with our consumers, their friends and families in their individualised recovery journey. The Role The Clinical Nurse Specialist - Discharge Planning will undertake the coordination of quality discharge planning for patients with severe and enduring complex mental health needs, developing change management strategies to facilitate timely access and streamlined patient flow. The Laboratory Assistant provides operational support for laboratory freezer inventory management. The role applies established procedures and system requirements to maintain accurate records in mySample, supports consistent storage practices, and contributes to the effective operation of freezer inventory systems.
