This position will support both the Program Coordinator and the BHH Care Coordinators to obtain, input, and report patient and provider data. The candidate will support the Hub Care Coordinator(s) by assisting with outreach to providers and/or patients/families to obtain necessary documentation such as screeners/rating scales, Mental Health background forms, etc. Daily task completion and data entry into the Hubs’ REDCap database is required. Report compilation, as requested, to meet all submission deadlines to DOH and the subcontracted Program Evaluator, on a weekly, monthly, and quarterly basis.
