Job Description:

The Family Support Worker (FSW) assists overburdened families residing in Alachua, Columbia, Union, & Bradford counties in improving family functioning and raising healthy children, by advocating and supporting these families with personal and parenting needs. The Family Support Worker meets regularly with the client in her home. (Weekly visits initially decreasing to every two to four weeks once the family is well established in pursing their goals.) In order to provide adequate support, the Family Support Worker’s caseload is 15-25 families. Ensure ongoing programmatic adherence to UF and Healthy Families of Florida (HFF) [Ounce of Prevention] standards.

Duties to include:

  • Establish a trusting relationship with the client and family by maintaining regular home visit as required per client level. Maintain dependable, punctual work schedules with a minimum of 80% completed home visits per level requirement. Assist strengthening parent/child interaction by providing in home education, utilizing established program curriculum and by modeling appropriate behavior during each home visit.
  • Initial contact with newly assigned families will be made within two working days with continued creative outreach until family is enrolled or closed.
  • Establish a family support plan with goals, objectives, and activities with supervisor and client within 90 days of participant agreement. This plan directs every home visit and is reviewed and updated at least every 180 days.
  • Meets with supervisors weekly (1.5 hours minimum) for case management of all families on caseload. Documentation of observations, interactions, and teaching completed at each home visit must be completed and submitted to supervisors within 72 hours of making home visit. All other forms must be completed on time per written protocol.
  • Schedule and coordinate referrals of high-risk families to other agencies to provide needed services to each family. Prepare documentation of results for supervisor’s review, and files in client’s HFA chart.
  • Supports families who are scheduled for family court proceedings by attending such hearings when the family wishes. Prepare documentation of results for supervisor’s review, and files in client’s HFA chart.
  • Administer Adult/Adolescent Parenting Inventories to all families identified by supervisor and/or High Risk Specialist as needing this additional assessment. Prepare documentation of results for supervisor’s review, and files in client’s HFA chart.
  • Attend “Front Burners Club” meetings as needed to internally staff needs of highest risk families. Prepare documentation of results for supervisor’s review, and files in client’s HFA chart.
  • Administer Home Safety Checklists with all families, as per schedule, and provides associated teachings as required to include (but not limited to): Shaken Baby, co-sleeping, water hazards, emergency preparedness, and poison prevention. Prepare documentation of results for supervisor’s review, and files in client’s HFA chart.
  • Demonstrate excellent knowledge of community resources, make referrals, and provides follow-up with families to insure needs are met. Maintain appropriate communication with community agencies and providers.
  • Attend regular staff meeting and case conferences as required. Participate in continuing education training and shares knowledge with other team members. Assists and support ongoing program growth and development.
  • Perform QA audits for families as directed by supervisor to update family data.
  • Perform other related duties as required.
Advertised Salary:

$15.00 per hour

Minimum Requirements:

High school diploma or equivalent and one year of experience working with children or families. Must have valid driver's license, reliable vehicle, and current car insurance.

Preferred Qualifications:
  • High school diploma or equivalent and 18 months experience as a Family Support Worker;
  • Two years experience working with infants and their families;
  • Ability to communicate effectively verbally and in writing;
  • Ability to organize time and activities;
  • Ability to work well independently and function appropriately with long-term, ongoing projects;
  • Ability to demonstrate respect and sensitivity for the rights of others;
  • Ability to be a good listener and non-judgmental;
  • Ability to foster self-sufficiency and independence;
  • Ability to follow written and verbal instructions;
  • Ability to work in partnership with a client;
  • Ability to establish and maintain working relationships with others;
  • Believes in and is comfortable with advocating for nurturing, nonviolent discipline of children;
  • Bilingual fluency in English-Spanish.
