Job Description:

The Royal Hobart Hospital and Hospital@home department is seeking interest from suitably qualified applicants for a Staff Specialist – Geriatric Medicine vacancy - 0.6 FTE (45.6 hours/per fortnight)

Who are we?

The Royal Hobart Hospital (RHH) is the largest hospital in Tasmania and the major referral centre for numerous tertiary services for the state. As the major clinical teaching and research centre, the RHH collaborates with the University of Tasmania and other institutions.

The Hospital@home service provides care to a range of medical, surgical, and sub-specialty patients who require acute and subacute inpatient medical management that can be delivered in the community environment.

The Hospital@home service was started in April 2020 and currently has 22 beds, 14 acute & 8 subacute Geriatric Evaluation and Management (GEM) beds. We also have a Rapid Response service that assesses community patients to prevent hospital admission. The service runs 7-days per week. At present we assess and admit around 200 patients a month. Our average length of stay of 5-6 days for acute patients and 13 for subacute patients.

The service is currently undergoing an expansion process and by the end of 2024 should have 30 beds – 18 Acute & 12 Subacute GEM beds. Our case mix and the age of our patients is broad. Half our patients are admitted from the Royal Hobart Hospital, both inpatient wards and the emergency department. The other half are admitted directly from the community avoiding hospital entirely. These patients are cared for in community health centres, their homes, or residential aged care facilities.

The interdisciplinary service is comprised of Medical staff, Nurse Practitioners, Nurses, Pharmacists, and Allied Health Professionals, who all work together to deliver effective clinical care.
