Job Description:

As a Community Health Worker (CHW) in our Office of Community Outreach & Engagement (COE), you will be responsible for providing relevant and timely resources and information directly to community members to improve health behaviors targeted at preventing cancer, as well as provide resources for those diagnosed with cancer, family members, and other cancer caregivers as a liaison between the COE and the communities across our catchment area.

This is a community-facing, professional position, ideal for candidates with experience or an interest in community health and developing a career as a professional CHW. Individuals in these roles are critical to forming relationships of trust between the Center, clinics, and the community and essential to addressing cancer-related health disparities in our communities.

In your new role you will be responsible for the following:

  • Forming community partnerships to direct service support, educational materials, and clinical navigation activities to the community in support of locally tailored programming.
  • Conducting outreach in community-based settings to raise awareness of COE initiatives and Project Continuity services and other UFHCC opportunities for care.
  • Promoting and implementing current health centered programming as well as developing and establishing new community health educational programs.
  • Coordinating community-based referrals, advocating for identified community needs, and distributing health information, home-based screening kits, and other resources throughout the community.
  • Participating in ongoing CHW, chronic disease, cancer-prevention trainings, and professional development opportunities in pursuit of a Florida accredited CHW certification.

This position reports to our Assistant Director of Clinical-Community Navigation as part of our larger Office of Community Outreach & Engagement. Selected candidates will be expected to achieve their CCHW certification within 6 months of hire, with training and support from the COE. Expected Salary:

$25.00 – $28.00, hourly; Commensurate with education and experience.

Minimum Requirements:

High school diploma or equivalent and one year of relevant healthcare experience; or an equivalent combination of education and experience.

Valid Florida Driver's License.

Preferred Qualifications:

Knowledge/Experience:

  • Associate’s Degree in Basic Sciences, Health Sciences, or similarly relevant field.
  • Certification as a Medical Assistant, LPN, or similar with community health experience.
  • General familiarity with the community resources, social services, and healthcare systems in rural communities, specifically focused on central and north-central Florida.
  • Demonstrated experience with community-based advocacy services and the willingness to act as a community advocate.
  • Professional experience working with diverse ethnic, socio-economic, and educational populations in a community health and/or clinical environment.

Skills:

  • Excellent oral and written communications skills, especially proof-reading and public speaking skills.

  • Demonstrated collaborative skills with internal and external stakeholders to strategically plan deliver assigned projects.
  • Bilingual proficiency in Spanish and English.
  • Ability to properly demonstrate and communicate health education and/or self-monitoring activities.
