Job Description:

The Role:
  • As part of Healthcare Connect North, review inpatients at the Launceston General Hospital (LGH), and existing clients of Primary Health North Community Services, that are frequent users of the public health service. In collaboration with members of the LGH Integrated Operations Centre team identify patients that would benefit from additional supportive services to reduce their length of stay in hospital.
  • Within a Primary Health Care Model, and as part of Healthcare Connect North, participate in a multidisciplinary, nurse led program to support access to relevant community services and intensive coordination of care for high users of hospital care.
  • Supports the efficient and effective provision of care, based on clinical standards and best practice principles within a collaborative and multidisciplinary framework to support individuals with complex care needs in the community.

The Tasmanian Health Service seeks a General Practitioner or Medical Specialist to join our newly established team, Healthcare Connect, which is be based in Launceston.

The General Practitioner or Medical Specialist will be part of a multidisciplinary team which will support individuals with complex chronic conditions to receive timely and intensive community-based care. The service will enable high users of hospital care to receive the coordinated and comprehensive care that supports them to remain in the community.

The General Practitioner will review patients allowing access to timely and intensive multidisciplinary community-based care, seamlessly integrating patient care from the public health system to targeted complex care in the community.

As part of Healthcare Connect North contribute to the development of care plans that meet the patient’s needs in the community, with ongoing review and sharing that information with other members of the health team involved in the client’s care.

  • Participate in daily multidisciplinary team meetings.
  • Establish interactive and networking relationships with General Practitioners including the GP Liaison Consultant, Nursing, Allied Health Professionals, and other relevant health care providers responsible for the ongoing care of clients.
  • Ensure patient clinical records are maintained (Medical Director, DMR, PAS) in an accurate, timely, and comprehensive manner and ensure the safe transfer of care when appropriate to other members of the health team, including General practitioners and Medical Specialists.
  • Work collaboratively with the project evaluation team to develop and implement data collection, monitoring and evaluation systems and contribute to project reports as required.
  • Actively participate in and contribute to the organisation’s Quality & Safety and Work Health & Safety processes, including the development and implementation of safety systems, improvement initiatives and related training.
  • The incumbent can expect to be allocated duties, not specifically mentioned in this document, that are within the capacity, qualifications and experience normally expected from persons occupying positions at this classification level.

Details of Appointment

Fixed term, part time, day work position working up to 40 hours per fortnight commencing as soon as possible until the 30th of June 2024.

Specialist Medical Practitioner Level 1- Senior Specialist Medical Practitioner Level 3 $163,005 - $247,768 per annum, pro rata

Please note the access to the Senior Specialist Medical Practitioner salary range $234,726 -$247,768 is subject to the successful applicant satisfying the criteria for Senior Specialist Medical Practitioner Level 1, 2 or 3 as outlined in the Medical Practitioners (Tasmanian State Service) Award

In addition, the following allowances calculated on the salaried incremental point may be available:

  • Provision of a fully maintained private plated motor vehicle or motor vehicle allowance of $25,758 per annum
  • Communication allowance of $1948 per annum
  • Continuing Professional Development Allowance of $24,927
  • Access to salary packaging
  • Relocation package and accommodation allowance available
  • Market Allowance of up to 35%


Successful applicants will be required to meet the essential criteria:

  1. Specialist or limited registration with the Medical Board of Australia in a relevant specialty
