Job Description:
The Department of Health is seeking interest from suitably qualified applicants for a Team Leader – Continuing Care vacancy.
Please note this position is also being advertised as a Nurse Unit Manager (Team Leader) – Continuing Care (510935m) and only one position will be filled on merit.
Continuing Care Team - South
- This position is based in the Continuing Care Stream (CCS) – South and will operationally manage a community team working with adults with complex and enduing mental health concerns in a trauma informed manner. This is a multi-disciplinary team with strong administrative support. The position is one of 3 based in the south so you will work closely with the other two team leaders to ensure consistency across the CCS.
The Team Leader - Continuing Care, as part of a multidisciplinary team delivering high quality mental health services in accordance with the Mental Health Services (MHS) National Mental Health Standards, Agency policy, legal requirements and relevant professional competencies, will provide leadership and direction in the strategic development and management of a comprehensive multidisciplinary community continuing care team (CCT).
As a member of the MHS South Group Management Committee work effectively in the implementation of strategic direction, review and evaluation of services, development of policies and improvement of business practices across the Southern area.
The Role:
- Provide strategic leadership and management of a MHS multidisciplinary community CCT to ensure the provision of high quality, comprehensive, specialist mental health service within a specified regional area in accordance with the Continuing Care Stream Operational Service Model and related documentation. This includes leading and managing the coordination of overall patient care within the designated service.
- Provide effective management of allocated human, physical and financial resources. This also includes planning, organisation, preparation and analysis of budgets, service resources and staff appointments.
- Provide leadership and change management to facilitate the implementation of the new model of care as specified in the Continuing Care Stream Operational Service Model and related documentation and optimise outcomes for service consumers and their families/carers.
- Ensure appropriate resources are met for intake, crisis response, assertive case management and regional group program requirements through the implementation and management of designated case allocation tools and evidence based best practice.

Details of Appointment:
Permanent, full time, day work, position working 76 hours per fortnight, commencing from as soon as possible.
*notwithstanding hours may be negotiated with the successful applicant

Location:
Mental Health Services - South
Base Salary: $129,849 to $131,355 per annum + 10.5% superannuation
- Salary range is in accordance with Allied Health Professionals Public Sector Unions Wages Agreement 2019
In addition, the following allowances calculated on the salaried incremental point may be available:
- Professional Development Allowance of up to $724 per annum with the ability to accrue fund up to five years
- Post graduate qualification salary advancement where applicable
Eligibility:
Successful applicants will be required to meet the essential criteria:
- Registered with the Occupational Therapy Board of Australia; or
- Degree in Social Work giving eligibility for membership of the Australian Association of Social Workers; or
- Registered with the Psychology Board of Australia
