Job Description:

Salary
$96,689 to $114,391 pro-rata, per annum
Please note that access to salaries beyond $105,245 is subject to completion of a relevant Graduate Diploma or application to the personal upgrade scheme in accordance with Schedule 8 of the Allied Health Professionals Public Sector Unions Wages Agreement 2019

Period of Employment
Multiple, permanent, full-time & part-time, day work, working up to 76 hours per fortnight, commencing as soon as possible.
Fixed term, full-time, day work, working 76 hours per fortnight, commencing as soon as possible, for approximately 12 months.
Please note that actual hours may be negotiated with the successful applicant.
Please also note that this process may be used to fill fixed term and casual
opportunities as they arise.

Duties
Within a primary health care framework assess the needs of older persons and appropriate younger people with disabilities and their carers, within the region. Facilitate access to appropriate community and residential services, through a multi-disciplinary team approach.
Essential Requirements
- Degree in Social Work giving eligibility for membership of the Australian Association of Social Workers; or
- Registered with the Occupational Therapy Board of Australia; or
- Registered with the Psychology Board of Australia; or
- Registered with the Physiotherapy Board of Australia.
- Current Driver's Licence
The Head of the State Service has determined that the person nominated for this job is to satisfy a pre‑employment check before taking up the appointment, on promotion or transfer. The following checks are to be conducted:
1. Conviction checks in the following areas:
a) Crimes of Violence
b) Sex Related Offences
c) Serious Drug Offences
d) Crimes involving dishonesty
2. Identification check
3. Disciplinary action in previous employment check
Desirable Requirements
- Holds or is working towards a qualification in the human services area which includes units of case management/case work practice
