Job Description:

The Role:
The Tasmanian Health Service seeks a Medical Specialist to join a new and innovative service, Healthcare Connect, which will be based in Launceston.
The Medical Specialist will be part of a multidisciplinary team which will support individuals with complex chronic conditions to receive timely and intensive community-based care. The service will enable high users of hospital care to receive the coordinated and comprehensive care that supports them to remain in the community.
The Medical Specialist will review patients allowing access to timely and intensive multidisciplinary community-based care, seamlessly integrating patient care from the public health system to targeted complex care in the community.
As part of Healthcare Connect North contribute to the development of care plans that meet the patient’s needs in the community, with ongoing review and sharing that information with other members of the health team involved in the client’s care.
- Participate in daily multidisciplinary team meetings.
- Establish interactive and networking relationships with General Practitioners including the GP Liaison Consultant, Nursing, Allied Health Professionals, and other relevant health care providers responsible for the ongoing care of clients.
- Ensure patient clinical records are maintained (Medical Director, DMR, PAS) in an accurate, timely, and comprehensive manner and ensure the safe transfer of care when appropriate to other members of the health team, including General practitioners and Medical Specialists.
- Work collaboratively with the project evaluation team to develop and implement data collection, monitoring and evaluation systems and contribute to project reports as required.
- Actively participate in and contribute to the organisation’s Quality & Safety and Work Health & Safety processes, including the development and implementation of safety systems, improvement initiatives and related training.
- The incumbent can expect to be allocated duties, not specifically mentioned in this document, that are within the capacity, qualifications and experience normally expected from persons occupying positions at this classification level.

Details of Appointment
Fixed term, full time, day work, position working 76 hours per fortnight, commencing 27 June 2022 until 28 June 2024

Salary:
Specialist Medical Practitioner Level 1 - Senior Specialist Medical Practitioner Level 3 $159,262 - $242,079 per annum
Please note that access to the Senior Specialist Medical Practitioner salary range $229,337 -$242,079 is subject to the successful applicant satisfying the criteria for Senior Specialist Medical Practitioner Level 1, 2 or 3 as outlined in the Medical Practitioners (Tasmanian State Service) Award
In addition, the following allowances calculated on the salaried incremental point may be available:
- Provision of a fully maintained private plated motor vehicle or motor vehicle allowance of $24,346 per annum
- Communication allowance of $1903 per annum
- Continuing Professional Development Allowance of $24,355 per annum
- Access to salary packaging
- Relocation package
Eligibility:
Successful applicants will be required to meet the essential criteria:
Specialist or limited registration with the Medical Board of Australia in a relevant specialty
Applicants should note the following criteria are desirable:
Fellowship of the Royal Australian College of General Practitioners.
